1/19/2024 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted JaCaeri upon arrival to the home. QMHP-C arrived to the home to conduct an individual session with JaCaeri. Upon arrival to the home setting, JaCaeri appeared to be in a good mood. QMHP-C informed JaCaeri that this session was going to focus on being able to interact with his peers and others in an appropriate manner without the use of verbal and physical aggression with behaviors lasting for about 20 minutes in duration. 2. Behavior: JaCaeri appeared to be receptive to the interventions that were provided to him as well as him presenting cooperative behaviors throughout the session; however, at times, he needed a little redirection from QMHP-C as it relates to his verbiage at times with behaviors lasting for 10 minutes in duration. 1.Intervention: QMHP-C began the session by exploring JaCaeri’s emotional wellbeing. QMHP-C explored with JaCaeri and JaCaeri’s mother JaCaeri’s impulsivity and how it affects JaCaeri’s relationships with her family members. QMHP-C encouraged JaCaeri to utilize “stop, think and reflect” techniques before making decisions. QMHP-C explored with JaCaeri and family several scenarios and encouraged JaCaeri and family to utilize Situation, Options, Disadvantages, Advantages and Solution (SODAS) to determine what to do. 2. Intervention: QMHP-C educated JaCaeri on impulse control strategies and praised JaCaeri and family for their participation in the session. QMHP-C facilitated a therapeutic game for JaCaeri and family to work on together. QMHP-C ended session by reviewing interventions with JaCaeri and encouraging JaCaeri and family to provide feedback. 1. Response:. JaCaeri appeared to be receptive to counselors’ interventions and services that were provided to him during the session. While exploring JaCaeri’s emotional state, JaCaeri expressed to QMHP-C that he was in a good mood. JaCaeri stated that he was excited because he wanted to show QMHP-C what he has been writing in his journal since the last session. JaCaeri’s mother stated that she was upset because she had concerns about her job and the financing when it comes to her obtaining different housing for her and her children. JaCaeri’s mother stated that JaCaeri and his siblings often do things without thinking about the consequences and that she is consistently yelling at them to clean up and stop arguing with each other. While exploring “stop, think and reflect techniques”, JaCaeri’s mother stated that this would be helpful if JaCaeri and his siblings would use it. 2. Response: JaCaeri and family actively participated and gave several options for solutions to the situation. During the therapeutic activity JaCaeri and his siblings were yelling at each other, pushing each other’s hands out of the way and arguing. JaCaeri’s mother stated that this resembled how they interact with each other in the home.
